Parents/Guardians.
As the new school year starts, I am requesting that you provide pertinent updated medical information (Allergy action plans, seizure action plans, asthma action plans etc.)
If your child has any epi-pens, inhalers, daily medications, emergency medications that they will take while at school, please make sure you provide those ASAP and ensure you have filled out the medication administration consent form.
